Have you been banned before: Yeah, but in jailbreak.

Other: I was banned in jailbreak a long time ago.. so i banevaded before i knew what it means... so at the time i was banned from jailbreak a few months ago Cowboy banned me. so the first ban i got in surf is for banevading. but i didn't in surf ever get banned for a valid reason. Ray unbanned me in jailbreak but i think i didn't mention me joining surf while that time... sorry for troubles please unban me from all servers if you are willing to.

Please remember that Admins are only required to keep the demo for a duration of 7 days. After that they are not obliged to keep it any longer. You will also need to upload the automatic screenshot made just before the ban was enforced. You can find this screenshot in your cstrike folder, under the format '[mapname]00*'. Example: de_dust2001. You should attach this immediately in your unban request as it will help you and ourselves in determining the case more effectively.

If you have used cheats, you should admit it in your first post. If you admit you have used any program or setting in order to gain advantage over others, your ban length might be softened and your request will be reviewed faster.[/roundframe]

just keep in mind getting banned from one Ambrosian Server = getting banned from all Ambrosian servers.
The screen-shot can be found in your Cstrike folder, you should provide it to help us help you more.
other than that just wanted to clear our something to you , if you were not unbanned officially by having filled up a form and got the Unban from the Unban-panelists, then you have never got unbanned from ambrosia, good luck
